Which form should I sign?
If you plan on getting permanent jewelry, be sure to sign the Permanent Jewelry Waiver.
If you are getting customized jewelry WITHOUT undergoing the permanent jewelry process, sign the Policy Form instead.
You do NOT need to complete both.
